This web site is to provide you with a process to validate eligibility and order replacement HP NC364T PCI Express Quad Port Gigabit Server Adapters. Please limit orders to parts you are prepared to immediately install and return the defectives within five days.
When you receive your replacement parts, please install them as soon as possible. Please retain the original packaging material as you will need to return the parts in the same packaging that it was received in.
A prepaid "Return Document" is included with each box received. Please return the parts removed from your system as soon as possible.
